Neurology International 2010; volume 2:e17 Correlation of cumulative corticosteroid treatment with magnetic resonance imaging assessment of avascular femoral head necrosis in patients with multiple sclerosis

چکیده
Increased risk of osteoporosis, fractures, and avascular necrosis (AVN) has been suggested in multiple sclerosis (MS). Patients with MS are often exposed to corticosteroid treatment (CST) during the disease course and conflicting reports exist regarding complications of CST. Our study aims to investigate the association between cumulative doses of CST and radiographic evaluation of AVN of the femoral head in MS. Twenty-six MS patients (mean age, 38.4±10 yr) were enrolled and prospectively evaluated for AVN by magnetic resonance imaging (MRI). The mean disease duration was 11.5±8.5 years and mean expanded disability status scale (EDSS) score was 3±2. The cumulative dosage of CST varied between 20 g and 60 g; patients were grouped into two categories: 1) CST between 20-40 g, 17 (65%) patients; 2) CST ≥40 g; 9 (35%) patients. The relationship between cumulative CST dosage and MRI diagnosis of AVN was stat istically insignificant (P>0.9). Clarifi cation of the cumulative effect of CST in the development of AVN is of great importance for future long-term steroid treatment strategies.

متن کاملCLINICAL CORRELATIONS BETWEEN AUDITORY BRAIN STEM RESPONSE AND MAGNETIC RESONANCE IMAGING IN PATIENTS WITH DEFINITE MULTIPLE SCLEROSIS
In an attempt to assess objectively the integrity of the auditory pathways in 30 patients with definite multiple sclerosis (MS), an audiometric evaluation was performed and auditory brainstem responses (ABRs) were obtained. Stressing the auditory system by increasing the stimulation rate showed some enhancement in the identification of MS. 24 (RO%) patients had an abnormal ABR along with c...
متن کاملPrediction of posttraumatic femoral head osteonecrosis by quantitative intraosseous aspirate and core biopsy analysis: a prospective study.
Twenty-two patients with unreduced hip dislocation or fracture dislocation were prospectively evaluated. Intraosseous aspiration, marrow fluid analysis and core biopsy histological analysis was performed from the supero-lateral (test group) and central part (control group) of the femoral head. After appropriate surgical treatment and postoperative management, they were followed up for 2 years b...
متن کاملAvascular necrosis of the femoral head presenting as trochanteric bursitis.
Five patients are described with avascular necrosis of the femoral head who presented with ipsilateral trochanteric bursitis, in the absence of clearcut hip joint disease. Avascular necrosis was indicated by magnetic resonance imaging. It is suggested that clinical trochanteric bursitis, especially when refractory to local corticosteroid treatment, may be the initial sign of hip disease. In the...
متن کاملMagnetic Resonance Imaging of Central Nervous System and Paranasal Sinuses in Multiple Sclerosis Patients: Findings from a Survey of Clinical Records in Kermanshah Province
Background and Objectives: Multiple sclerosis (MS) is the most common demyelinating disease of the central nervous system with more than 5.2 million people across the world being afflicted with. Magnetic Resonance Imaging (MRI) is a valuable tool in the diagnosis of MS. This study surveys the results of MRI of the central nervous system and paranasal sinuses in the sample of MS patients in Ker...
